Clinical Experiences of Free Flap Transfer

Abstract
Since the introduction of microscope in surgery by Jacobson and Suarez in 1960 -many surgeons have tried to transfer the free flap using microvascular anastomosis in animal, and the first successful microvascular free flap in human was performed by Harii in 1972. Nowadays, the distant transfer of a large composite tissue by microsurgery has been popularized in the field of plastic and reconstructive surgery.
It has many advantages over the conventional pedicle flap.
They are as follows:
1) This method saves time with a single operation and short hospitalization.
2) It improves the comfort of the patient and allows early ambulation.
3) It provides a well vascularized flap, because it is an "arterial flap" based on a
well defined arteriovenous pedicle.
4) It diminishes the risk of donor site infection because the donor site is primarily closed.
5) The complete skin cover obtained allows bone, tendon and nerve surgery to be performed primarily.
During the past two years (Sept. 1978Feb. 1980) authors experienced 8. cases of -free flap transfer from groin, deltopectoral region, axilla and dorsum of the foot in the patients of severe scar contracture of axilla, arm and hand, hypertrophic scar of face and soft tissue loss of a heel. Among them 6 cases were successful, 2 cases were failed probably due to errors in the , flap elevation.
In this paper, we would like to report our cases with brief discussion.
